Should I contact ERL® in case of an emergency?

No. If you are experiencing an emergency, or if there is an immediate danger to life or property, do not rely on ERL®. Immediately contact the local authorities or dial 911.



What is ERL?

ERL® is The Human Equation's Electronic Report Line, a third-party, independent reporting system designed to allow you to report anonymously and confidentially any incidents, serious wrongdoings, or crimes that you have observed.



Can I report an incident anonymously?

Absolutely. ERL® does not require you to leave a name or any other contact information in order to report an incident. You need only your Access Code to report an incident.



What are the different ways I can use ERL to report an incident?

ERL® offers you 4 ways to report an incident: online, by phone, by fax, or by mail.



Will I be contacted personally after I report an incident?

That depends. If you report the incident anonymously, then you will not be contacted because your organization has no way of knowing your identity. If you do leave contact information, then you may be contacted if it is determined that contacting you directly is appropriate or necessary under the circumstances.



How can I report an incident?

Reporting an incident through ERL® is simple and convenient. You always have 4 options available to you, anywhere and anytime:
  1. Online: Access our secure, Web-based incident report form at www.electronicreportline.com;
  2. Telephone: Call our automated, toll-free hotline number, 24 hours a day, 7 days a week, at 1-877-GET-EARL (438-3275);
  3. Fax: Fax your incident report (INCLUDING your Access Code) to our toll-free, dedicated fax-line server at 877-382-2810; or
  4. Mail: Send your incident report (INCLUDING your Access Code) to us at:

Where do I find ERL on the Internet?

You can find ERL® at www.electronicreportline.com. From there, just follow the directions that appear on each screen.



What is my Access Code?

Your Access Code is a number that allows us to identify your organization. Since you are not required to give your name or any other identifying information, your organization's Access Code allows us to forward your incident report to your organization. Regardless of which method you use to report an incident, your Access Code MUST be provided to us.



What if I do not have Internet access or do not feel comfortable reporting an incident online?

No problem. There are three other ways to report an incident. You can phone, fax, or mail your incident report to ERL. We make sure your information gets to the appropriate person.



How do I call ERL?

ERL® provides an automated, toll-free hotline for you to report an incident, in English or Spanish, at any time, from any phone. Simply dial ERL®'s dedicated number, 1-877-GET-EARL (438-3275), and follow the instructions.



How do I fax an incident report to ERL?

ERL® provides a toll-free hotline that allows you to fax your incident report to us from anywhere, at any time. Simply prepare your written incident report in any format you feel comfortable with, and fax it to 1-877-382-2810. You do not have to include your name or any other personal identification. Be sure to include your organization's ACCESS CODE and the name of your organization on your correspondence. Without this information, we have no way of identifying your organization or where to send the report.



How do I mail an incident report to ERL?

Mailing your incident report to ERL is no different from mailing any other letter. Simply prepare your written incident report in any format you feel comfortable with, and mail it to:

You do not have to include your name or any other personal identification on your incident report. Be sure to include your organization's ACCESS CODE and the name of your organization on your correspondence. Without this information, we have no way of identifying your organization or where to send your incident report.



How do I report an incident in Spanish?

If you choose ERL®'s online option, you can click on the EspaƱol link to access directions in Spanish. When you dial ERL®'s dedicated number, 1-877-GET-EARL (438-3275), you can choose to receive instructions and report incidents in Spanish. Incidents reported by fax or mail can be sent to ERL in Spanish.



Do I have to report the same incident more than once?

No. Once you have reported an incident, it will be forwarded to the correct person within your organization, and that person will determine the next course of action. Reporting the same incident more than once may cause unnecessary delays, so doing so is not recommended. However, if you learn of, witness, or are a victim of another incident in the organization, you should consider reporting the new incident through ERL®.



How soon can I expect a response?

ERL® makes certain that your incident report is submitted to your organization, assuming you have identified your organization by name or access code. How soon you receive a response to your incident report depends on your organization's internal policies and procedures. You should also be aware that due to the nature of some incidents, or depending on your organization's internal policies and procedures, you may not receive a response at all.



Can I get in trouble for intentionally submitting false or misleading reports?

Yes. The filing of false or misleading reports is strictly prohibited and may result in civil or criminal penalties.



Will the police get involved?

Your incident report will be submitted by ERL directly to your organization's designated recipient. The police or other authorities are not automatically notified of the information contained in your incident report. However, depending on the nature of the information in the incident report, as well as your organization's internal policies and procedures, the information you provided may ultimately be made available to the authorities.



Will there be legal action taken?

Any actions taken following your reporting of an incident depend entirely on your organization's own internal policies and procedures. It is up to your organization to decide the appropriate course of action following an incident report, which may or may not include some form of legal action.



Should ERL be the first step I take to report an incident in my organization?

Although ERL® is always available, you should first consider talking to the person in your organization responsible for addressing such concerns, so that the incident can be resolved in the most prompt and efficient manner possible. However, when speaking to someone within your organization is impossible and you require a more confidential means of reporting a problem, you may choose any one of the 4 reporting methods ERL® offers.
